Grooming Agreement

Current Vaccinations/Veterinarian Information:

By signing this agreement, owners verify their pet(s) is current on Rabies, Distemper and Parvo-Virus vaccinations. Veterinarian information, proof of a current Rabies Vaccination or current Titer testing is required prior to beginning any grooming services. Special circumstances will be taken into consideration.

Aggressive or Dangerous Pets:

Owners MUST inform Canine Creations Pet Salon & Spa if their pet(s) bites, has bitten, or is aggressive to people, other pets or during specific grooming procedures. Muzzles and/or cones may be used if necessary. Muzzling will not harm your pet and protects both the pet and the groomer. Canine Creations Pet Salon & Spa reserves the right to refuse/stop services for such pet(s) at any time before or during the grooming process. A special handling fee of $10 - $20 may be added to the regular grooming charge.

Health or Medical Problems & Senior Pets:

Grooming Procedures can sometimes be stressful, especially for a senior pet or a pet with health problems. These procedures can expose hidden or aggravate known medical problems, during or after the groom. Because senior pets with health problems have a greater chance of injury these pets will be groomed for cleanliness and comfort, in the style that will not add to their stress. In the best interest of your pet, this agreement will give Canine Creations Pet Salon & Spa permission to obtain immediate Veterinary treatment for your pet should it be deemed necessary by Canine Creations Pet Salon & Spa. We will do our best to contact you first, then take your pet to your authorized Veterinarian or to the nearest available veterinarian. It is agreed that all expenses for Veterinary care will be covered by the pet’s owner upon signing this agreement. Canine Creations Pet Salon & Spa may require review and acceptance of a separate Senior or Infirm Pet Release Form prior to performing services. 

We reserve the right to stop or refuse service of any Senior of Infirm pet if we believe the health and safety of the pet will be jeopardized.

Mat Removal: 

 Pets with matted coats need extra attention during their grooming session. Mats left in a pet’s coat will only grow tighter, which can strangle the pet’s skin or cause tearing. Heavy matting can also trap moisture and urine near the pet’s skin allowing mold, fungus or bacteria to grow, causing skin irritations that existed prior to the grooming process. Torn skin from mats can also harbor maggots. Canine Creations Pet Salon & Spa does not wish to cause serious or undue stress to your pet, and will not continually de-mat your pet for you. If we feel the matting is too tight or too extensive throughout the coat, we may require the pet to be shaved, which would be discussed and agreed to prior to starting. A shave down to remove a heavily matted coat includes risks of nicks, cuts or abrasions due to warts, moles or skin folds trapped in the mats. After effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to regrow. Shaved pets are also prone to sunburn and should either have sunscreen applied daily or should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases, pets can also exhibit brief behavioral changes. Canine Creations Pet Salon & Spa may require review and acceptance of a separate Matting Acknowledgement or Shave Down Release prior to performing services. Mat removal and/or shave downs will incur additional charges on top of the base grooming price of $10 - $45, depending on the severity of the matting and overall condition of your dog’s coat.

Accidents:

Grooming equipment is sharp and there is always the possibility an accident could occur. Even though we use extreme caution and care in all situations, possible problems could occur, including cuts, nicks and scratches. In most cases, this can happen when a pet is wiggling or moving around. We will notify all clients of any minor accidents that may occur during your pets grooming session. In order to decrease the chance for accidents, please do not try to see your dog and/or talk to them if you arrive for pickup and they are still being groomed. This will make the dog excited and that excitement can increase the possibility of an accident. If you insist on talking to your pet prior to the groom being completed, we reserve the right to end the grooming session, even if not completed, and the full grooming price will be charged.

Parasites:

If you suspect your pet has fleas or ticks, please tell us during the check-in process. If fleas or ticks are found during the grooming process, we will immediately treat your pet with a flea bath to kill the parasites. This is to protect both your pet and other pets in our care. By signing this agreement, you herein authorize said flea treatment should fleas be found during the grooming session and agree to pay the additional $15 fee for said treatment. Ticks found will be removed for an additional charge. If ticks are found we strongly suggest that you have your pet tested for Lyme Disease. Parasites are a health hazard to your pet, as well as to humans.

Hold Harmless Agreement:

By signing this agreement you (or your Agent) agree to hold Canine Creations Pet Salon & Spa, it’s owners, operators, employees, officers and directors harmless from any damage, loss or claim arising from any condition of the listed pet(s), either known or unknown, to Canine Creations Pet Salon & Spa. It is also further understood and agreed that the terms of this agreement can change at any time, without notice, and will override any and all prior signed contracts or releases. It is further understood that this clause applies to any and all pets groomed at Canine Creations Pet Salon & Spa.

Early Drop-offs & Late Pick-Ups:

We accept pets on an appointment-only basis (except for walk-in nail trims). We are not a boarding facility and have limited kennel space to hold animals while they wait for grooming to either be started or completed. We will text and/or call you upon the completion of your pet’s grooming appointment. We ask that you pick up your pet within ONE HOUR of this notification. After one hour, Daycare  fee of $25 will be added to the total price of your groom. A late pickup fee of $40 will be charged if pickup is later than one hour and after our closing time of 6pm.

No Shows, Cancellations & Late Drop-offs:

We require 24-hour notice of any appointment cancellations. After one No Show or last-minute cancellation, a $25.00 Deposit will be required for booking. We understand there may be emergency situations and will work with you, but not on a continued basis. Please be respectful of our time as we are a by-appointment business, and another client could have taken your appointment, if we had known.

 Photo Release:

We often take pictures of the dogs in our care and publish these on our social media sites such as Instagram and Facebook. By signing this agreement, you are granting Canine Creations Pet Salon & Spa, agents and employees the right to take, edit, alter, use and publish photographs and videos of your pet(s) for us in print, social media, and electronic advertising. No financial compensation of any type will be given with the taking or publication of these photographs and videos.
